- IRS SS4 Form
- Each office will have to work with the Organizational Leader to fill out the EIN application which is online. You will need the Full Legal name and SSN for the Organizational Leader to apply. The EIN Verification Letter must be emailed to Joy Bauder and Trent Hollister. Club will not be activated until received at the State 4-H Office.
